The following are the downloadable forms that supplement your e-Portfolio. Please review them either prior to or at the beginning of a given block.
LEARNER CARD (optional download)
This card is meant for a medical student learning from the R2 regarding the SIMPLE PROCEDURE. It is not required, but the attending may find this useful in gathering feedback.
There are no downloads for this block presently.
This is a worksheet that will prepare the resident for a subsequent shift where he/she can discuss the case with the SAME attending (usually this will be the preceptor). They are to audit the outcome (e.g. progress during an admit or discharge) of a patient for whom they have arranged admission.
Residents are WARNED, again, NOT to put personal patient information (e.g. name) other than what is suggested on the worksheet. Once the task is complete, this form should be destroyed.
Multi-source Feedback cards (GIVE 5 to RNs/Allied Health)
You will need to print 5 of these The attending should distribute these to 5 members of allied health and collect them back.
The attending should then read and SUMMARIZE key points made into the Task Assessment Form for the day.
